01Who it's for
Most people with diabetes benefit from specialist input. The program is open to:
- Have Type 1, Type 2, or gestational diabetes
- Have prediabetes with metabolic risk factors
- Want to optimise an insulin pump or CGM
- Need a second opinion on therapy choices
- Are an Ontario resident with a valid health card
02How the program works
Initial Intake & Records Review
Complete a short intake form online. Send recent lab results, A1C history, and current medications. Our team reviews within one business day.
Lab Work if Needed
Your physician may order updated labs — A1C, fasting glucose, lipids, kidney function, microalbumin — to inform your treatment plan.
Specialist Consultation
A virtual consultation with your DOS endocrinologist to review labs, discuss goals, and design a personalised plan.
Therapy Adjustments & Prescriptions
Medication starts or changes, CGM/pump setup, dose titrations — sent directly to your pharmacy.
Ongoing Follow-Up
Regular virtual check-ins (typically every 3 months) to review CGM data, adjust therapy, and monitor for complications.
03What's included
04Coverage & what's billed
What's covered: physician consultations, requisitioned lab work, in-store vitals review, prescription refills written by your DOS physician, and follow-up visits during the program.
What may be patient-pay: certain weight-loss or specialty medications (depending on private insurance), supplements, and any out-of-program services you elect. Your DOS physician will discuss any patient-pay item with you before prescribing — nothing is added to your file without consent.
05Your Metro pharmacist
Each Metro location partnered with DOS has at least one pharmacist briefed on the program. They have access to your DOS chart (with your consent) and can answer questions about side effects, interactions, and adjustments without you needing to wait for a physician appointment.

This program may not be right for you if you:
- Are experiencing diabetic ketoacidosis (call 911)
- Have severe acute hypoglycaemia (urgent in-person care)
- Need a hands-on physical exam for diabetic complications
