The following guidelines help indicate whether our weight management program may be appropriate. A physician assessment will confirm eligibility.
- Have a BMI ≥ 27 with weight-related health concerns
- Have Type 2 diabetes or prediabetes
- Have tried diet and exercise without sufficient results
- Are an Ontario resident with a valid health card
- Are able to participate in virtual care appointments
02How the program works
Complete Online Intake
Complete a short intake form online. Our team reviews your information and confirms next steps, typically within one business day.
Physician Consultation
A secure virtual consultation with your DOS physician to review your history, discuss your goals, and assess suitable treatment options.
Individualized Assessment & Care Plan
Your physician designs a tailored weight management plan based on your assessment, including medication options where clinically appropriate, with Registered Dietitian support for nutrition and lifestyle.
Laboratory Testing or Additional Investigations (if required)
Where indicated, your physician orders baseline labs — metabolic panel, thyroid, glucose, lipids — to guide and refine your plan.
Ongoing Follow-Up & Program Support
Regular virtual check-ins to monitor progress, adjust dosing, review results, and provide continuous support throughout your program.
03What's included
04Coverage & what's billed
What's covered: physician consultations, requisitioned lab work, prescriptions written by your DOS physician, and follow-up visits during the program.
What may be patient-pay: certain medications (depending on private insurance), devices, supplements, and any out-of-program services you elect. Your DOS physician will discuss any patient-pay item with you before prescribing — nothing is added to your file without consent.
05Your pharmacy
Your preferred Metro pharmacy can assist with prescription fulfillment, medication availability, general pharmacy-related questions and product inventory inquiries. Clinical assessments, diagnoses, prescribing decisions and treatment plan modifications remain the responsibility of your physician.

This program may not be right for you if you:
- Are pregnant or planning pregnancy
- Have a personal or family history of MTC or MEN2
- Have Type 1 diabetes
- Have a history of pancreatitis
- Are currently on certain interacting medications
